WHAT IS CONSULTASSISTENT?
ConsultAssistent is a program your doctor uses to collect information about your health. You will receive a questionnaire through ConsultAssistent. The questionnaire includes questions about your complaint, pain or discomfort, and how the complaint is developing once treatment with the doctor is completed. You can complete the questionnaire at home via the Internet before your appointment with the doctor.
Your doctor will review your answers in advance or during your outpatient appointment. This gives you more time during your appointment with your doctor for detailed questions and additional information.
ConsultAssistent also collects fully pseudonymized insights into complaint and complaint development from other patients with similar complaints. These insights are used by your doctor to improve your treatment. As a result, you yourself also automatically help improve care.

HOW DOES THE CONSULTASSISTENT WORK?
ConsultAssistant is used by your doctor to support the different steps in your treatment:
Your first appointment at the outpatient clinic
A few days before your first outpatient appointment, you will receive an e-mail inviting you to complete a questionnaire. The e-mail will be sent to the e-mail address known to the hospital.
The first time you use ConsultAssistent, you first create an account. This account allows you to access your data at any time. Upon logging in, you will be sent an SMS code for verification on the phone number known to the hospital.
What does the questionnaire say?
The questionnaire can consist of several sections. For example, questions about your health complaints, your medical history and general information.
The number of questions you will be asked varies by department and complaint. It often takes 10 to 15 minutes to complete, but this can be more in some situations. More questions are then needed to get a good picture of your complaints.
You can stop at any time and continue later. Your answers will be saved; you don't have to start over.
Your second or next appointment at the outpatient clinic
A few days before your follow-up appointment, you will receive an email inviting you to complete a (condensed) follow-up questionnaire.
Again, different questions will be asked depending on the department where you have your appointment and your symptoms. Usually the questions are about how your symptoms have developed. You can also change your profile and medication use.
The questionnaire is short and usually takes only a few minutes.
After your treatment
A few weeks or months after your treatment, you will be asked a few questions about your symptoms. Your doctor uses this information to see the results of treatment on your complaint. These answers are used by your doctor to improve the quality of care.
FREQUENTLY ASKED QUESTIONS
It is possible that the e-mail ended up in your spam box of your e-mail program. If it has not, please contact the outpatient clinic where you have your appointment. The contact information is in your invitation e-mail. You may have the wrong e-mail address registered.
If you search your e-mail for key words such as the name of your hospital, you may be able to find it. If not, please contact the outpatient clinic where you have your appointment. The contact details are in your invitation e-mail. They can send you another invitation.
Your username can be found in your account confirmation email. If you no longer have it, you can retrieve your username in ConsultAssistent. The link to ConsultAssistent is in your invitation email.
Have you forgotten your password? In the login screen you can request your password again. You will then receive a separate e-mail with instructions.
Your user name and password are case-sensitive. If you still cannot log in, please contact the outpatient clinic where you have your appointment. The contact details are in your invitation e-mail.
If you change your telephone number, please inform the hospital where you have your appointment. The contact details are in the invitation e-mail. Only they are allowed to change your telephone number.
That's not a problem. You can simply continue filling out the questionnaire. You can ask during your outpatient appointment with your doctor for an explanation of what was meant by this question. If you misunderstood, the doctor can always change your answer.
You may have changed your appointment to a date other than the original. Is the date still not correct? Please contact the outpatient clinic where you have your appointment. If your appointment has already taken place, you do not need to fill in this questionnaire. You will receive a new invitation for your next appointment.
Check whether you have filled in all the required fields. If you still get an error message or the questionnaire still freezes, please check your internet connection and log off and on again.
Any answers you have entered before receiving the error message have been saved.

You will soon have an appointment at the outpatient clinic of your hospital. Your doctor uses ConsultAssistent to gain insights into your complaint and complaint development. He or she has sent you an invitation to fill in questionnaires via the internet.
With ConsultAssistent, you can better prepare yourself for the visit to your doctor. You can go through all the questions at home, so that during your appointment there is more time for additional or more in-depth questions. Your doctor has a better understanding of your symptoms and care requirements because of your answers. Moreover, ConsultAssistent gives you and your doctor insight into the course of your experienced symptoms. The treatment can then be tailored more adequately.
The completed questionnaires automatically appear in your Electronic Medical Record (EMR). This gives the doctor direct insight into your symptoms and their progress.
ConsultAssistent collects fully pseudonymized insights into complaints and complaint development from other patients with complaints similar to yours. These insights are used by your physician to improve your treatment. Researchers at Outcome Measurement (the supplier of ConsultAssistent) also conduct research on pseudonymized and anonymized data. They only use your data with your consent. In all research conducted by Outcome Measurement, care improvement is the goal.
Pseudonymisation is a security measure whereby personal data are processed without it being clear which persons the data refer to. When personal data is pseudonymised, it is only possible to trace the data back to a specific person if additional data is used.
Only the healthcare providers involved in your treatment have direct access to your personal and medical data. Researchers only have access to pseudonymised or anonymised data. This means that your data cannot be traced back to you.
ConsultAssistent is safe and reliable and meets the strictest standards regarding storage and processing of medical data. ConsultAssistent works in compliance with the AVG legislation and according to the NEN 7510 standard for information security in healthcare. In addition, ConsultAssistent has a CE mark for medical devices.
