From patient journey to practical care pathway

Digital innovations only work if they fit the practice of the department as well as the patient experience. That's why we start each implementation by working out the care path together, as a first step to really get a good picture of the care.
Inspired by the patient journey template of the NVZ | Dutch Association of Hospitals, we developed a template for this purpose.
A practical tool for mapping out the care path step by step with the professional group:
